Recognizing mania at the bedside — and the medication that requires some of nursing's most precise safety monitoring

Mania presents in ways that can look almost like the opposite of what nurses expect from a psychiatric patient — high energy, confidence, and talkativeness. Recognizing it accurately, and managing lithium safely, are both distinctly tested skills.

Before We Start
Why mania can be easy to under-recognize

Mania doesn't look like most people's mental image of psychiatric distress — patients often appear energetic, confident, even euphoric. This can make it easy to under-recognize as a symptom requiring intervention, rather than simply "a good day." Understanding the specific DIGFAST symptom cluster helps distinguish genuine mania from ordinary high spirits or a good mood.

💡 Hypomania vs. Full Mania
Hypomania is a less severe form — no psychosis is present, and hospitalization typically isn't needed. Full mania, by contrast, can include psychotic features and poses genuine safety risks requiring more intensive intervention. The DIGFAST symptoms apply to both, differing mainly in severity and functional impact.

D — Distractibility
Attention easily pulled to irrelevant stimuli
The patient's attention shifts rapidly and is easily drawn away from the task or conversation at hand.
I — Impulsivity/Indiscretion
Poor judgment in decision-making
Risky spending, impulsive sexual behavior, or other poor-judgment decisions made without the usual consideration of consequences.
G — Grandiosity
Inflated self-esteem or sense of importance
An exaggerated sense of one's own abilities, importance, or capabilities — in severe mania, this can escalate into grandiose delusions.
F — Flight of Ideas
Rapid, loosely connected thoughts
Thoughts move quickly from one topic to another, sometimes connected only by superficial associations (like rhyming or a tangential idea) rather than logical connection.
A — Activity Increased
Increased goal-directed or purposeless activity
The patient may take on multiple projects at once, or show generally increased physical restlessness and activity.
S — Sleep Decreased
Reduced need for sleep, not just reduced sleep
Critically, this isn't insomnia in the sense of wanting to sleep but being unable to — the patient often feels rested and energized on very little sleep, which is itself a distinguishing feature.
T — Talkativeness
Pressured speech
Rapid, forceful speech that can be difficult to interrupt — a classic, often immediately noticeable sign of mania on a bedside assessment.
💊 A patient who reports feeling fully rested on 2-3 hours of sleep, combined with several other DIGFAST symptoms, should raise concern for mania rather than simply being viewed as having unusually good energy.

Practical nursing strategies
Offer high-calorie finger foods — manic patients are often too busy and distractible to sit through a full meal, but need adequate nutrition. Simplify the environment to reduce excess stimulation. Use matter-of-fact, consistent limit-setting on behavior, without engaging in arguments. Monitor closely for safety, since poor impulse control can lead to risky behaviors. Watch for signs of exhaustion, since manic patients can push their bodies well past normal limits without recognizing the need to rest.
Lithium — Therapeutic Range
0.6–1.2 mEq/L
Lithium is the first-line mood stabilizer for bipolar disorder, but has a narrow therapeutic window requiring regular level monitoring.
Lithium Toxicity Progression
Recognizing escalating severity
Early signs include a fine tremor and gastrointestinal upset. As toxicity worsens, signs progress to a coarse tremor, ataxia (unsteady gait/coordination), and in severe toxicity, seizures.
The Sodium Connection
A critical, non-obvious safety detail
Sodium depletion — from dehydration, diuretic use, or excessive sweating — increases lithium toxicity risk, because lithium and sodium are handled similarly by the kidneys; when sodium is low, the body retains more lithium. Maintaining adequate, consistent sodium and fluid intake is therefore a genuine, specific safety teaching point for any patient on lithium.
🏥 Clinical Scenario — A Lithium Patient With a New GI Illness
A patient stable on long-term lithium therapy develops vomiting and diarrhea from a viral illness over the past two days, and reports a new, noticeably coarser hand tremor.
Connect the Findings
Vomiting and diarrhea cause fluid and sodium loss — and because sodium depletion increases lithium toxicity risk, the new coarse tremor is a meaningful warning sign, not just an incidental finding alongside an unrelated stomach bug. The GI illness and the tremor are directly connected through this sodium-lithium relationship, not two separate, unrelated problems.
Escalate Appropriately
The nurse holds the next lithium dose pending provider input, requests a lithium level, and ensures the patient is rehydrated with attention to adequate sodium replacement. Continuing lithium as usual during a period of significant fluid and sodium loss risks allowing toxicity to progress further, especially with early signs (coarse tremor) already present.
Ongoing Teaching
Once stable, the nurse reinforces the specific teaching point that any illness causing significant fluid or sodium loss (vomiting, diarrhea, excessive sweating, or starting a diuretic) requires prompt attention and possibly provider contact for a patient on lithium. This isn't a one-time teaching point — it's an ongoing, situational awareness the patient needs to carry into future illnesses.

Nutrition strategy: "What nutritional approach is appropriate for a patient experiencing acute mania?" → High-calorie finger foods that can be eaten on the go, since the patient may be too distractible to sit through a full meal.
⚠️ The Trap — Not Connecting a GI Illness to Lithium Toxicity Risk
Vomiting and diarrhea are common, usually unremarkable illnesses — which is exactly why the connection to lithium toxicity risk can be missed in a patient who happens to also be on lithium therapy. Treating the GI illness in isolation, without considering its effect on lithium levels, risks allowing toxicity to progress undetected.

The safeguard: Any significant fluid or sodium loss in a patient on lithium — from illness, medication changes, or environmental factors — should prompt consideration of lithium level monitoring, not just symptomatic treatment of the illness itself.
