GTPAL is easy to recite as five letters, but recording it correctly requires precisely understanding what counts toward each category — particularly the distinction between Gravida (all pregnancies) and the four categories that follow, which break down the OUTCOMES of those pregnancies rather than counting pregnancies again.
💡 Two Specific Definitional Details Worth Getting Right
Gravida counts ALL pregnancies, including the CURRENT one if the patient is currently pregnant. Abortion includes BOTH elective and spontaneous (miscarriage) pregnancy losses — it isn't limited to elective termination the way the term is sometimes used colloquially.

This is a count of pregnancies, not outcomes — a patient pregnant for the third time, regardless of how her first two pregnancies ended, has a Gravida of 3.
T — Term
Deliveries at 37+ weeks
Counts births that reached full term specifically.
P — Preterm
Deliveries between 20–36 weeks
Counts births that occurred before full term but after the threshold that distinguishes a preterm birth from a pregnancy loss.
A — Abortion
Spontaneous or elective, before 20 weeks
Includes both miscarriages (spontaneous) and elective terminations, occurring before the 20-week threshold that would otherwise classify the loss as a preterm birth.
L — Living
Number of children currently alive
Reflects current living children — distinct from the number of deliveries, since twins/multiples or a child's later death would create a difference between total births and living children.

Determine Gravida
This is her 4th pregnancy overall, including the current one — so Gravida = 4. Gravida counts pregnancies, and the current pregnancy is included in that count.
Classify Each Prior Pregnancy Outcome
One full-term delivery → Term = 1. One delivery at 34 weeks (between 20-36 weeks) → Preterm = 1. One first-trimester miscarriage (before 20 weeks) → Abortion = 1. Each prior pregnancy outcome is classified into exactly one of these three categories based on the gestational age at which it ended.
Determine Living
Both of her previous live-born children (from the term and preterm deliveries) are currently alive → Living = 2. The complete GTPAL notation for this patient is G4 T1 P1 A1 L2.

GTPAL questions test both the definitions and correctly calculating a specific patient's notation:
Definition application: "Does Gravida include the patient's current pregnancy?" → Yes.
Category classification: "A pregnancy loss at 22 weeks — does this count as Preterm or Abortion?" → Preterm (the 20-36 week range covers this specific gestational age, distinct from Abortion, which is before 20 weeks).
Full notation: "A patient currently pregnant for the second time previously delivered one full-term baby who is currently alive, with no other pregnancy losses. What is her GTPAL?" → G2 T1 P0 A0 L1.
⚠️ The Trap — Miscounting Gravida as Only Prior (Not Current) Pregnancies
Because T, P, A, and L all describe outcomes of PAST pregnancies, it's an easy mistake to apply that same "past only" logic to Gravida as well — undercounting by one if the patient is currently pregnant.
The safeguard: Remember specifically that Gravida includes the current pregnancy if one is in progress, while the other four letters describe completed pregnancy outcomes only.
